1. This procedure applies not only to the isolation of the energy drive source such as electricity to electric motors, but to the isolation of all other energy sources that could cause an injury, such as gases or liquids under pressure or trapped in lines or vessels, and radioactive sources.
2. Selector switches, push buttons, or other control circuit type devices are sufficient energy isolating devices.
3. Every person working on the isolated equipment must have full personal protection against the release of hazardous energy through the use of a personal lock and tag.
4. Energy isolation locks for Operations are_____________________ in color.
5. Contractors performing work at the Neal site will supply their own safety-type padlocks which must be of a different color than Braskem’s personal locks
6. Each contract worker, working on the job, must attach their personal lock and tag to the lockbox. Each Contract person working on the job must sign on the back of the field copy of the permit and post it at the work location.
7. One authorized person of each crew/craft, working on the isolated equipment, must install a craft lock and tag on the lockbox; and if applicable, one on the Klein Clip in the MCC building. The “craft lock” must remain on the lockbox until all work by that craft is complete.
8. Any person (Contract or otherwise) performing work on, or entering the hazard zone of the locked out piece of equipment, including supervisors, engineers, etc., shall verify that a permit is present, and place their personal lock on the lockbox.
